Admissions

 Referrals must have:

•  Application
• Current IQ Scores
• Current Psychological Evaluation - (not more than 3 years old)
• PCP Referral - (Primary Care Physicians) 
• Social Security Card, Birth Certificate, and Medicaid Card
• Documentation of past Mental Health Treatment - (Outpatient and/or Acute Services)
• Recommendation for residential treatment by outpatient provider

PRTF referrals may have:

• Previous acute care treatment for stabilization
• Failed in the outpatient treatment setting 
• Displayed non-compliant/disruptive behaviors in the home and/or school, or have violated court orders
• Failed in other mental health treatment settings.
